About the Program

Downtown Atlanta, Inc.’s Supportive Services Homeless Outreach Team is dedicated to engaging individuals experiencing unsheltered homelessness with compassion, dignity, and respect. Through proactive outreach, trauma-informed care, and resource navigation, our team works to connect individuals and families with shelter, supportive services, housing opportunities, and pathways toward long-term stability.

Working in partnership with service providers, government agencies, businesses, and community stakeholders, we help coordinate solutions that address both individual needs and broader community concerns. Our goal is to improve quality of life, enhance public spaces, and foster a safer, more inclusive Downtown Atlanta for all.

Services

The following services are offered directly and/or through coordinated referrals from agencies within the District.

Core Street Outreach Services
Proactive canvassing and engagement Wellness checks and basic needs assessment Resource navigation and warm handoffs to partner agencies
Stabilization and Navigation Support
Interim shelter placement support Documentation readiness (ID, birth certificate, Social Security card) through partner coordination
Transportation and “Pathway Home” Support
Transportation assistance to support access to medical appointments and employment (MARTA, Lyft rides) Reunification support through the Pathway Home program, including contacting family or support networks, verifying destination stability, coordinating travel logistics, and providing basic travel essentials such as hygiene kits and snack packs.
Coordinated Entry and Housing Navigation Support
Coordinated Entry assessment (VI-SPDAT) and documentation for housing queue placement Referrals to diversion and other problem-solving pathways when appropriate Coordination with Partners for Home and housing providers on housing referral timelines and documentation requirements

Our Approach

The Homeless Outreach Team is dispatched through direct requests from Downtown Atlanta stakeholders, as well as through proactive, on-the-ground engagement and problem-solving interactions with individuals experiencing unsheltered homelessness throughout the Downtown Improvement Distric

Resource Brochure

We have created a Supportive Services Resource Brochure as a tool to assist persons in need of shelter, medical, ID, and food resources. Please keep the brochure as a reference and stay tuned for opportunities to learn, participate in conversations, and join the collective effort to support the most vulnerable among us with dignity and respect.

Monthly Social Services Provider Forum

The Interagency Social Services Forum (ISSF) is a monthly, informal convening of City of Atlanta social service agencies. The forum provides a space for partners to network, share resources, exchange ideas,, and provide updates on available programs and services for individuals experiencing unsheltered homelessness and other forms of housing instability.

ISSF strengthens coordination across service providers to improve access, awareness, and delivery of support for those in need throughout the community.

Resources

The Street-to-Home initiative is a partnership with the United Way of Greater Atlanta designed to support individuals transitioning from unsheltered homelessness into permanent housing.

This move-in assistance program helps cover critical one-time costs, including security deposits, first month’s rent, and utility activation fees, providing up to $2,000 per household. By reducing financial barriers to housing placement, Street-to-Home helps accelerate exits from homelessness and supports long-term housing stability for individuals and families.

In addition to housing placement support, the program also connects individuals to Pathway Home reunification services, helping those who wish to return to supportive family or community networks by coordinating travel, verifying safe destinations, and assisting with the logistics needed to successfully reunite.
